Unity Care is a strength-based and culturally proficient youth and family development agency. For 27 years, we have been delivering supportive services to underserved and foster youth and families in Santa Clara, San Mateo, San Francisco, Alameda, Solano, Placer, Sacramento and El Dorado counties. With a multicultural staff of 56 employees, Unity Care serves transitional age foster youth ages 16-21 and homeless adults. Mission: Unity Care’s mission is to provide quality youth and family programs for the purpose of creating healthier communities through lifelong partnerships. Our goals are to provide safe, secure, and positive living environments; educate and support under served youth; and prepare young people to pursue professions in which minorities are chronically underrepresented. Who We Serve - Unity Care housing and programs serve transitional age foster youth (TAY) and families ages 16–21 in Santa Clara, San Mateo, San Francisco, Placer, and El Dorado counties. Our signature program, founded in 1993, focuses on serving the housing needs of youth in foster care and beyond. Our Philosophy - At Unity Care, we focus on meeting individualized needs and working together for the benefit of each child and family in our care. We put our philosophy into practice through our 1-Child 1-Plan framework, which promotes effective and efficient teamwork through one service delivery plan for each youth or family referred to our care.
