PCHS provides the community with skilled health care professionals who work together for the betterment of our patients' overall health. We are a certified Patient-Centered Medical Home and an accredited federally qualified community health center. PCHS is a nonprofit 501(c)3 organization. Our Administrative Office is located in Bremerton, WA with our clinics located throughout Kitsap County WA and Mason County, WA. We use an integrated care model, collaborating with our internal teams for the best possible patient outcomes. Our diverse provider team includes primary medical care, behavioral health, on-site pharmacy and preventive/restorative dental care. By working together, we are able to address the whole care/whole needs aspect of our patients and community. Also in our 30+ years in the community, we have developed strong partnerships with external providers and community agencies to provide referrals to outside agencies when our patients' care needs extend beyond our doors. As of 2018, PCHS provides care to 29,000 patients at seven medical clinics and five dental clinics. We were the first community dental clinic in the country to co-locate a dental clinic on the campus of a community mental health center and are continually striving to break down barriers to care for all members of our community, whether it is access, coverage, language, etc. Our latest projects include a major remodel of our flagship downtown Bremerton clinic (6th Street) that began in September 2018, our first school-based clinic at the North Mason School District in Belfair opened in September 2018 and our fifth dental clinic opened in October 2018 in Poulsbo, bringing much needed dental services to North Kitsap county. In our near future we have mobile clinic, bringing primary medical and behavioral health clinic to those who are unable to access care in our brick and mortar clinics.
